Job summary:

GluCare Integrated Diabetes Center is seeking a dedicated and DHA-licensed Assistant Nurse to support our nursing team in delivering high-quality care to patients managing diabetes and related health conditions. The Assistant Nurse will assist in providing direct patient care, supporting clinical procedures, and ensuring smooth operation within the center. This role requires a compassionate individual committed to supporting patients on their healthcare journey while working collaboratively with a multidisciplinary team.

Key Responsibilities:

Patient Care Support:

1. Assist registered nurses and physicians in providing direct patient care, including vital sign checks, blood glucose monitoring, and patient observations.
2. Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans.
3. Administer basic treatments and support with clinical procedures under the supervision of a registered nurse or physician.

Clinical Procedures Assistance:

1. Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.
2. Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection.

Patient Education and Communication:

1. Communicate effectively with patients, providing empathy and clarity to address their questions and concerns.
2. Reinforce health education provided by registered nurses and physicians, encouraging self-care practices and adherence to treatment plans.

Documentation and Reporting:

1. Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR).
2. Report any changes in patient condition to the supervising nurse or physician promptly.

Operational and Safety Compliance:

1. Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well-being of patients and staff.
2. Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.

Desired Candidate Profile

Qualifications:

1. Valid DHA Assistant Nurse License - A MUST
2. Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management.
3. Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred.
4. Strong communication and interpersonal skills with a patient-centric approach.
5. Basic proficiency in electronic medical records (EMR) systems is preferred.
