City Health Federation is currently recruiting an experienced, enthusiastic, forward-thinking Health and Wellbeing Coach on behalf of Horton Primary Care Network (PCN6) - a network of nine practices and 63,000 patients - working collaboratively across the network area with our community partners to look after the health and wellbeing of our local population. The role provides a great opportunity to join a committed team.

You will be working alongside a team of a social prescribing link worker, a care coordinator, and a Mental Health Practitioner, taking referrals from GPs and other professionals in the network. You will spend time with the people referred to you, building trusting relationships, listening carefully to what matters to them and what motivates them, and supporting them to take action on their self-identified goals.

We are looking for a highly motivated individual who will work as part of a multi-disciplinary team to provide expertise across the PCN.

You will be a central point of contact for patients, linking them with sources of support within local communities. You will take a person-centred approach to work with patients identified by GP surgeries and undertake guided conversations with patients.

You will be expected to develop links with local GP practices, colleagues within the service, local communities, and VCS to ensure delivery of integrated, patient-focused care, demonstrating flexibility in the role, and a caring attitude towards patients.

This is an exciting position and you will need to be self-motivated and able to work on your own, whilst also integrating into a number of well-established General Practices. You will have excellent organisational, interpersonal, and communication skills together with a good working knowledge of primary care and the challenges facing patient care in the NHS.

Job purpose

Working alongside a social prescribing link worker and care coordinator, taking referrals from GPs and other professionals in the network. The post holder will spend time with the people referred, building trusting relationships, listening carefully to what matters to them and what motivates them, and supporting them to take action on their self-identified goals.

Key responsibilities:

1. Work with GPs and the wider clinical team to identify and manage a caseload of patients likely to benefit from health and wellbeing coaching, taking referrals from practices within the PCN for individuals who would benefit from support.
2. Work closely in partnership with the Social Prescribing Link Worker to identify and work alongside people who may need additional support but are not yet ready to benefit fully from social prescribing.
3. Manage and prioritise a caseload, in accordance with the health and wellbeing needs of the local population through taking an approach that is non-judgemental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues.
4. Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit most from health coaching.
5. Provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently.

• Minimum 1 year relevant experience (voluntary, informal or paid) of supporting people in either a health, social care, public health or learning improvement field.
• Experience of using coaching approaches/frameworks and models or other helping strategies e.g., motivational interviewing.
• Knowledge of public health issues in the local area.
• Experience of delivering support programmes on a group basis.
• Experience in supporting people with their mental health, physical wellbeing and supporting people living with a learning disability either in a paid, unpaid or informal capacity.
• Experience of working in a multidisciplinary team.
• Experience of working in health and social care/community development setting or similar.
• Networking experience.

• Interpersonal and communication skills both verbal and written.

• Ability to prioritise and effectively manage own workload.
• Ability to deal with conflict in a professional manner.
• Able to build and sustain relationships at all levels.

• Excellent group and one-to-one facilitation skills including conflict resolution.
• Experience of electronic patients records, particularly SystmOne.

• GCSE Grade C (or equivalent) in Maths and English.
• Educated to NVQ Level 3 or equivalent.
• Be enrolled in, undertaking or qualified from Personalised Care Institute accredited health coaching training.
• Educated to degree level.
• NVQ Level 3 or equivalent in coaching, mentoring or one of the health priority areas.
• Qualified Assistant Practitioner in relevant field.
• Coaching/counselling qualification/experience or other relevant qualification/experience involving reflective listening skills.

• Flexibility to work outside of core office hours if required.
• Reliable.
• Ability to work under pressure and meet deadlines.
• Well organised.
• Ability to travel across all sites.

Salary: £26,500 to £28,000 a year, dependent upon previous experience.
