Vitalis are recruiting for an experienced Qualified Social Worker to work in a Preparing for Adulthood Team in Bromley.

Responsibilities

As a Qualified Social Worker, you will:

• Work collaboratively with young adults and their families to support their transition into adulthood.
• Assess needs and provide appropriate interventions and support.
• Work within a multi-disciplinary team to ensure effective service delivery.
• Maintain accurate and up-to-date case records.
