This is an exciting opportunity for an enthusiastic and highly motivated psychologist to be part of a specialist Mesh Centre multidisciplinary team (MDT). Services for women with complications of mesh inserted for urinary incontinence and vaginal/colorectal prolapse are now organised nationally. NBT is the centre for the South West.
The psychologist will be supporting the lead psychologist in this urogynae MDT, offering consultancy, training and supervision, attending joint clinics, assessment and interventions for women in the service. The post-holder will encourage the team to care for patients in a Trauma‑Informed way.
The psychologist will support patients with complex surgical decision‑making and be confident working with pain distress, adjustment issues and delivering complex‑trauma interventions pre/post mesh removal & other surgeries.
The post-holder will be part of the Clinical Health Psychology department, within the surgery division and make good links with Pain Management colleagues and become familiar with suitable treatment pathways. They will link in with psychologists working in Mesh Services across the UK.
The psychologist will be supervised by a BABCP accredited/EMDR Consultant which provides an excellent opportunity to achieve accreditation or EMDR practitioner/consultant status.

Main duties of the job
- To provide brief assessment of all patients coming into the Bristol Mesh Service at the OneStop Clinic.
- To participate in MDT processes, actively contributing to discussion and treatment planning.
- To provide specialist psychological training and consultation to the Mesh MDT (surgeons, specialist nurses, pain consultant, radiologists, clinical scientists, pelvic health physiotherapists & admin support).
- To support the team to consider how to work in a trauma‑informed way.
- To offer support to patients with complex decision making & consent issues. This may include, facilitating joint clinical appointments with other members of the team.
- To understand the psychological barriers in complex‑decision making and be able to provide interventions to help patients work through these.
- To offer pre‑surgery psychology interventions, post‑surgery inpatient support, and post‑surgery interventions as appropriate (e.g. EMDR for medical trauma).
- To be able to work effectively with patients demonstrating high distress, anger, pain and shame e.g. incontinence /psychosexual issues +/or avoidance behaviours.
- Liaise with psychology colleagues, such as in the Pain Management Service/in‑patient staff.
- To refer/liaise with other services as appropriate e.g. for risk/safeguarding concerns.
- Be involved in research and service evaluation.

Mesh Complications Specialist Service – South West
North Bristol NHS Trust provides a Mesh Complications Specialist Service on behalf of South West patients. Patients can travel from out of area and even abroad if they have suitable funding to be seen by our specialist team.
The multidisciplinary team (MDT) will assess and treat patients with complications of mesh inserted for urinary incontinence and vaginal prolapse, and where indicated will undertake mesh removal surgery. The MDT aims to provide support to this cohort of patients throughout their care pathway. Patients are followed‑up for 5 years within the service.

Knowledge, Training, Experience And Skills Required
- Doctoral Degree in Clinical or Counselling Psychology or other equivalent Psychological Therapy Training with relevant BPS & HPCP accreditation.
- Registered as a Practitioner Psychologist with the Health and Care Professions Council.
- Appropriate level of understanding of, and experience with, working with people with physical health issues, including in an acute hospital setting.
- Appropriate level of understanding of, and experience with, evidence‑based psychological models and therapeutic approaches to health and illness, (e.g. EMDR, Cognitive Behavioural Therapy, Compassion Focussed Therapy, Acceptance and Commitment Therapy).
- Experience of the clinical assessment of psychological functioning, cognitive abilities, mental health needs and capacity for therapeutically supported change using a range of strategies including clinical interview, patient reported outcome measures, and evaluation of information from other sources.
- To develop and maintain an appropriate level of understanding of MDT clinical practices in mesh surgery and associated issues possibly relevant for women coming into the service E.g. The Cumberlege Report; NHS England Mesh Working Group.
- A thorough knowledge of local & national mesh removal services scope and supporting documentation e.g. NICE patient decision aids.
- To develop and maintain expertise and experience in psychological trauma‑focused work, e.g. trauma‑focused Cognitive Behavioural Therapy / Eye Movement Desensitisation and reprocessing Therapy (EMDR).
- Expertise for training other health professionals, including senior colleagues.
- Expertise for supervising psychologists and other health professionals.
- Commitment to continuing professional development.
- Professional knowledge and expertise of psychometric practice. For example, scale development, use and evaluation within physical health settings and experience in the collection, analysis, and dissemination of audit and clinical data

Skills Required
- To co‑ordinate the Psychology Service input to the Urogynaecology Mesh service at North Bristol NHS Trust, with regard to supporting the development of identifying priorities, coordinating available resources, managing waiting lists, and ensuring appropriate clinical governance in all aspects e.g. with patient records.
- To communicate in a highly sensitive and skilled manner sensitive information to /with patients and colleagues in a compassionate and effective way, for example where there are high levels of distress, psychological risk, anger, or where barriers to self‑care exist.
- To formulate and contribute to the interdisciplinary care planning for patients, taking into account highly complex factors, and associated liaison with other parties e.g. the refers/ other relevant MDTs.
- To contribute directly and indirectly to a psychologically based framework of understanding and care to the benefit of all patients of the service, across all settings and agencies serving the patient group.
- To undertake risk assessment and risk management for individual patients, and to provide advice to other professions on psychological aspects of risk assessment and management.
- Ability to contain and work therapeutically with people in extreme psychological distress that patients may experience, including depressive withdrawal, anger, suicidality, anxiety / panic, loss and bereavement, relationship breakdown. This includes being skilled in providing trauma‑based interventions, pain management skills, and acceptance and adjustment work.
- Ability to put psychological theory into practice in novel and complex clinical settings, synthesising data and theory from a range of evidence‑based sources to create individual, group, team intervention plans.
- Ability to deliver training on psychological matters in clinical settings at a variety of levels to colleagues and to evaluate / audit training.
- To develop and adapt the Psychology Service to the Urogynaecology Mesh service as appropriate to meet the needs of the Acute Hospital.
- Ability to evaluate and prioritise competing clinical demands and to use planned and ad hoc supervision as part of coping with these.
- To be involved, as appropriate, in HR matters, for example the appointment of psychologists/ psychological therapists / assistant / graduate psychologists.

Essential criteria
- Post qualification experience of teaching/training to healthcare professionals
- Advanced communication skills relevant to this multidisciplinary (MDT) setting and patient group. Capable of effective working in circumstances of high emotional distress and severe pain amongst patients.
- 1. Doctorial Degree in Clinical or Counselling Psychology or other equivalent Psychological Therapy Training with relevant BPS & HPCP accreditation.
- To be able to communicate at a high level with other professionals including surgeons with patients present in joint clinic setting
- Able to demonstrate high quality team working and communication skills
- To be able to work effectively with patients demonstrating high levels of distress +/or risk
- Recognise when need personal support / personal therapy or other professional support
- Make effective use of supervision
Desirable criteria
- Further training in Trauma Informed Practice & Interventions e.g. parts I, II, III EMDR
- Post qualification NHS experience of working with adults with longterm conditions e.g. chronic pain / health or surgical complications
- Post‑qualification experience of using psychological trauma interventions (trauma focussed CBT/EMDR)
- Experience of teaching delivered to multi professional groups
- Experience & Skills in research, audit & evaluation, including management of projects and managing research budgets.
- Doctorial Degree in Clinical or Counselling Psychology or other equivalent
- Post qualification experience in healthcare
