LifePath provides support services to older people, people with disabilities and caregivers. We are currently seeking an RN for the Home Care Program. The Home Care Nurse participates as a member of a collaborative Home Care team in the assessment and authorization of personal care plans for Home Care clients. In addition, the Home Care Nurse is responsible for nursing home pre-admission screening and determining the medical eligibility for other Medicaid funded services.
QUALIFICATIONS / REQUIREMENTS:
- Registered Nurse with valid Massachusetts license required. BSN from an accredited school and three years clinical experience preferred.
- Experience in gerontological nursing, community health nursing or long term care preferred.
- Excellent assessment and evaluation skills.
- Computer experience (word processing, data entry) necessary to process all preadmission screening requests and maintain client records.
- Ability to work independently and to work in an interdisciplinary team setting.
- Knowledge of community resources in community based long term care and the elder service network.
- Requires valid driver's license, insured automobile in good working condition, availability to work between the hours of 9:00 a.m. and 5:00 p.m., ability to work in the event of an emergency during off hours.
COMPENSATION AND BENEFITS:
$34/hr RN, 6% pay differential for bilingual skills. $5000 sign-on bonus payable in two equal installments at 3 months and 6 months of employment. Excellent benefits including medical, dental, vision, life insurance, 401k, generous paid time off, Flex Spending, and Employee Assistance Program.

ESSENTIAL FUNCTIONS:
- Conduct pre-admission screening of Medicaid clients seeking long term care and alternative community programs.
- Assess Home Care clients as referred for personal care and complete a personal care plan, making referrals for other programs as appropriate.
- Monitor the appropriateness of Personal Care Plans through home visits, telephone reassessments, or through case management assessments; at appropriate and compliant frequency.
- Maintain current list of Personal Care and Home Health Program clients and Respite Home Health and Personal Care Aides.
- Participate in monthly meetings with team Case Managers and Home Care Program Manager.
- Participate in case conferences and case reviews as requested.
- Participate in ongoing internal quality reviews.
- Develop and maintain collaborative relationships among community agencies, hospitals and other institutions that service medical and social needs of clients.
- Monitor clients on Home Health Program for continued appropriateness of service.
- Perform evaluations / reevaluations for Personal Care Assistance Program.
- Attend required Home Care and LifePath staff trainings and meetings.
- Conduct long term care insurance assessments.
- Stay abreast of developments in gerontological health issues, medication and long term care.
