What you will do:

1. Conduct assessments with individuals referred through the Liaison and Diversion service to understand their needs.
2. Develop and implement care plans in collaboration with multidisciplinary teams.
3. Provide support and interventions tailored to the needs of individuals with mental health, learning disability, or substance misuse issues.
4. Liaise effectively with police, courts, and other services to ensure coordinated care for individuals.
5. Maintain accurate records and ensure compliance with all relevant policies and procedures.
