Job Summary
We are looking for an experienced Paramedic Practitioner to join our new Enhanced Care & Frailty Team at Dover Town PCN. This is a multi‑site role delivering high‑quality, person‑centred care through home visits and care home support. You will play a key role in proactive care, working closely with our multidisciplinary team to improve outcomes and independence for patients across our PCN area.

Responsibilities
- Undertake pre‑arranged and same‑day visits to housebound community patients and patients in care settings.
- Provide clinical assessment, diagnosis and treatment within scope of practice, liaising with GP practice and other teams.
- Conduct regular visits to care and nursing homes, delivering proactive and responsive care in line with the EHCH framework, completing initial reviews within 7 working days for new residents and hospital discharges.
- Use sound clinical judgment, prioritise caseloads and refer for investigations as required.
- Use sensitive, person‑centred communication to ensure patient understanding and consent.
- Prioritise and manage workload independently while promoting high‑quality care.
- Promote self‑care, patient education and preventive approaches to health and wellbeing.
- Work closely with GPs, nurses, pharmacists, social prescribers, SECAmb, KCHFT, Frailty Teams, hospital and social care to deliver integrated, multidisciplinary care.
- Participate in weekly Multi‑Disciplinary Team meetings to support care planning for complex or vulnerable patients.
- Support proactive management of long‑term conditions, frailty and palliative care needs.
- Evaluate treatment plans and advise care home staff; use EMIS and other clinical systems to record interventions and support audit processes.
- Maintain professional registration and engage in ongoing CPD and supervision.
- Share knowledge and contribute to team development, participate in training and reflective practice.
- Promote equality and diversity, respect privacy, dignity and beliefs of patients, families and colleagues.
- Engage in annual performance reviews and maintain a professional development portfolio.
- Support innovation and develop new models of care in collaboration with colleagues.
- Post holder must be available to work weekends on a rota basis and be committed to working across multiple sites, completing home visits and regular visits to aligned care homes.

Qualifications (Essential)
- BSc in a training programme approved by the College of Paramedics.
- Active and relevant professional registration with HCPC.
- Completed two‑year Consolidation of Learning period as a "newly qualified paramedic" and have a further three years' experience as a band 6 (or equivalent) paramedic.
- Commitment to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of commencement.
Qualifications (Desirable)
- Evidence of verification against the paramedic primary care roadmap.
- Independent or supplementary prescribing qualification, or willingness to undertake.
Experience (Essential)
- Experience managing the care of patients presenting with undifferentiated conditions and a designated caseload.
- Experience in urgent, emergency, or community‑based care.
- Strong clinical assessment and decision‑making skills.
- Experience of working within a multi‑disciplinary team.
- Experience of working under own direction in a patient‑focused environment.
- Understanding of HCPC professional standards for practice.
- Compassionate, adaptable and community‑focused approach.
- Commitment to providing safe, effective, and person‑centred care.
- Ability to write comprehensive, accurate clinical notes, implement and evaluate care plans.
- Good IT skills, including ability to record accurate clinical notes.
- Excellent interpersonal, influencing and negotiating skills.
- Able to work under pressure and meet deadlines.
Experience (Desirable)
- Experience working within a primary care or care home environment.
- Evidence of working across organisational boundaries within health and social care.
- Knowledge of the Enhanced Health in Care Homes (EHCH) framework.
- Experience with EMIS.
- Knowledge of primary care networks and collaborative working.
