We are seeking qualified and experienced Dietitian to join the Allied Health team. Reporting to the Assistant Head, you will be required to assess, diagnose and provide nutritional care and guidance to dialysis patients, ensuring their nutritional needs are met accordingly to best practice standards. Your responsibilities are as follows:
1. Nutrition Screening and Risk Evaluation
- Conduct nutrition screenings using reliable and validated tools to assess patient risks and levels of criticality, guiding service delivery based on the findings.
2. Comprehensive Nutritional Assessment
- Collect and record pertinent data (medical, anthropometric, biochemical, social, and environmental) to create a tailored nutrition management plan.
- Conduct in-depth diet interviews to gather a complete diet history.
- Perform Subjective Global Assessment (SGA) and actively participate in interdisciplinary assessments.
3. Diagnosis and Nutrition Problem Resolution
- Apply dietetics knowledge and critical appraisal skills to diagnose nutrition-related problems and devise treatment plans or interventions.
4. Development and Implementation of Nutrition Plans
- Develop individualized nutrition plans, offering diet counselling and interventions tailored to the patient’s health and nutritional needs.
5. Ongoing Patient Monitoring and Follow-Up
- Monitor patient progress through follow-up sessions and modify nutrition care plans as needed to achieve optimal outcomes.
6. Contribution to Nutrition Programs
- Contribute to the development and execution of nutrition projects and programs (e.g., outreach initiatives, group education programs).
7. Professional Development and Knowledge Update
- Regularly review relevant literature, attend workshops, and pursue conferences to keep up-to-date with advancements in nutrition and dietetics.
8. Additional Duties as Assigned
- Perform other duties related to the nutrition care of dialysis patients as required by the healthcare team.
Qualifications
- Bachelor’s or advanced degree in Food and Nutrition or Dietetics. Completion of a clinical dietetics internship (at least 24 weeks) in an accredited institution.
- Membership in the Singapore Nutrition and Dietetic Association and accreditation as an Accredited Dietitian in Singapore (advisable).
Experience
- Minimum 1-2 years of clinical nutrition experience.
