ROLE PURPOSE
This position will provide outreach and in-reach services to individuals who are housed at the Bedford Manor Assisted Living facility. The position includes receiving referrals, completing intakes, teaching life skills, referring to community resources, crisis management, building skills in relapse prevention, and building support networks. Many of the residents have complex needs including physical health, social/emotional, and/or overlapping mental health and substance use and require support to navigate systems and maintain their housing in an Assisted Living Facility.

REPORTING AND WORKING RELATIONSHIPS
- Reports and is accountable to the Manager, Assisted Living or their designate
- Function as a multi-disciplinary team member both within Connective and externally with stakeholders
- Liaises with staff and volunteers of community and government agencies as required
- Liaises with the staff, managers, and on-call manager of Connective as necessary
KEY ACCOUNTABILITIES
- Monitor any changes in the resident that could impact their eligibility for Assisted Living by reporting the changes to the manager or their delegate.
- Collect relevant personal information from the resident, from family members or support persons, assessments provided by the health authority or other community partners and create a cohesive plan that identifies the types of services the resident will require to be successful in Assisted Living.
- Provide coordinated case management services by participating in meetings, supporting residents directly, and collaborating with funders and other identified interested parties to address needs related to maintaining their physical health, mental wellness, reducing illicit substance use, and providing opportunities for residents to engage in social activities, connect residents to emotional and community supports, personal care and personal living supports, food security supports, and indigenous cultural supports.
- As dictated by each supported individual’s needs and abilities, participate in the implementation and facilitation of programming that will allow each person to develop life skills such as nutrition and food preparation, banking, shopping, and recreational and social opportunities.
- Respond to supported individuals’ complaints or concerns and ensure documentation related to concerns or complaints is maintained to the standards of Connective, the health authority, and Assisted Living Registrar.
- Complete required reporting and participate in meetings with the health authority as scheduled.
- As part of a multi-disciplinary team, this position will respond, as and when needed, to support individuals at risk of losing their housing due to non-compliance with their residency agreement, mental health and substance use, or behaviour related to a complex mental health diagnosis by initiating interventions directly with the resident.
- This position will explore various housing options, discuss the behaviour and non-compliance that contributed to the risk of losing their housing directly with the resident, contact their family members or support people, and provide updates to the health authority and management team on any progress or solutions to maintain housing.
- Support individuals in obtaining and receiving medical, psychiatric, and physical health services by assisting with referrals, filling out paperwork, and/or advocating with the health authority and/or community partners to connect the resident to the appropriate resource.
- Network to build working relationships with community and health services providers including health authorities, acute care and community mental health and substance use support services.
- Make referrals to other programs or agencies as required.
- Model effective interactions with supported individuals, including positive rapport building and support to achieve their goals.
- Effectively utilize various agency systems and technology platforms.
- Support individuals in searching and securing alternate housing arrangements.
- Transport supported individuals to appointments or meetings with community partners, and between agency locations as required.
- Perform other related duties as required.
WORKING CONDITIONS
- Function independently, while remaining a crucial member of the support team.
- Adhere to Connective’s policies and procedures.
- Adhere to the Community Care and Assisted Living Act.
- Adhere to the regulations of the Assisted Living Registrar.
QUALIFICATIONS
- Diploma in Behavioural Science with an emphasis on Social Work, Seniors, Counselling or Mental Health and Substance Use; equivalencies may be considered.
- Experience providing 1-1 supports with individuals that struggle with mental health and/or intellectual and/or physical disabilities, interaction with social agencies and people living with psychiatric/personality disorders.
- Experience in case/care plan development and implementation.
- One (1) year recent related experience.
REQUIREMENTS
- Ability to successfully complete Criminal Records Checks.
- Current Emergency First Aid Certificate.
- Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements.
- Demonstrate clear ability to respond well in crisis situations.
- Excellent written and verbal communication skills in English.
- An understanding of providing service to people who are ageing and who may have different learning abilities.
- Class 5 driver’s license.
- Personal transportation that is in good repair, insured for business use with a minimum of $1,000,000 for liability, and which must be accessible when on shift.
Compensation
The wage range for this position is $27.54 to $31.61 per hour as per collective agreement.
