Position Summary

The position of the Housing and Program Initiatives Community Worker is responsible for participating in client program planning and program implementation and assisting in the pursuit of opportunities for community inclusion and participation. The position is also responsible for the implementation of clinical and professional recommendations in order to reach individual’s goals and maximize their potential.

Minimum Qualifications

  • 2 year Community College diploma, preferably BST, DSW, CYW
  • 1 year practical experience with individuals who have a developmental disability and/or dual diagnosis (some who may have come in contact with the law) in a community setting
  • Knowledge and understanding of the needs and best practice service standards for persons with developmental disabilities
  • Experience working in a residential behavioural setting is an asset
  • CPR and First Aid
  • Certification in non-violent crisis intervention (CPI)
  • Pharmacology training is an asset
  • Valid Ontario driver’s license and access to a reliable vehicle

Duties And Responsibilities

  • PROVIDES SERVICES TO CLIENTS BY:
  • Participating in the implementation of (clinical) approaches as recommended by the Individual Support Plan
  • Participating in the documentation of progress as it relates to day-to-day adaptive functioning in the home/community and in support of any treatment plans/recommendations
  • Assisting individuals to take part in their scheduled activities; this includes aiding and directing the individuals in their hygiene, housekeeping and educational routines
  • Participating in the provision of learning opportunities within the residential environment and the community as determined by the Individual Support Plan
  • Assisting individuals to access services and supports such as medical appointments, recreation opportunities and community events
  • PROVIDES ADMINISTRATIVE SUPPORT BY:
  • Participating in team meetings; sharing information with other staff
  • Participating in the administering of medications as required and maintaining written records as needed
  • Participating in routine maintenance and cleanliness of the short-term treatment home
  • Participating in data collection as required
  • Completing required program documentation
  • PARTICIPATES IN OTHER RELATED DUTIES INCLUDING (BUT NOT LIMITED TO):
  • Maintaining program materials, equipment, program areas, supplies, etc
  • Maintaining petty cash (up to $200.00)
  • Maintaining food inventory in accordance with the Canadian Food Guide
  • Maintaining monetary program requirements

  • Is knowledgeable about, ensures others are knowledgeable about and complies with the agency’s policies and procedures
  • Operates, at all times, in a safe and secure manner
  • HEALTH AND SAFETY:
  • Ensure Health and Safety Program requirements are followed based on the Occupational Health and Safety Act
  • Ensure an awareness of safety measures for the agency, yourself and the individuals receiving services
  • Ensure compliance with the agency’s mandatory training requirements

Status: Permanent Full-time
Hours of work: 72 hours bi-weekly
2025 Salary: $30.55/hourly - $33.99/hourly
Location: Brockville, Leeds Grenville
