Job Introduction
We have an exciting opportunity in our City Community Team to employ a Health Care Support Worker. Our HCSW are expected to work seamlessly within the multi-professional community team within the city locality and to forge close links with GPs and other local teams. We are an integrated co-located multi-disciplinary team which creates a wrapped around approach in the delivery of excellent patient care to our diverse population. We have good working relationships with our GP colleagues and secondary care. Within our team, we have nurses, care coordinators, community therapists, and Home First Rehabilitation Support Workers.
The community team can treat any patient over the age of 18 that is most appropriately seen in the home environment.

The successful candidate will be able to provide clear evidence of up-to-date, relevant clinical practice and education. Applicants need to have well-developed communication skills, good IT knowledge, and evidence of basic clinical skills. We are looking for an enthusiastic, motivated, and hardworking individual who can work flexibly and efficiently. The team operates various shift patterns from 07.00 until 22.00, 7 days a week.
For this post, you need to have a UK Driving licence and access to your own car.
