Join us and be part of our overnight nursing service! Help us shape and develop the service. We have an exciting opportunity for a Band 3 Healthcare Support Worker or a candidate that would be willing to work towards a Band 3, to join our team in Devizes, for 20 hours and 30 minutes a week.

Here at Wiltshire Health and Care, we are very excited to announce the development and expansion of our community services in line with the NHS Long Term Plan and a number of strategic priorities for community health services to transform our “out of hospital” care. We are boosting the response capacity within the existing community teams to provide a dedicated resource across each of our localities within Wiltshire.

This growth within our services incorporates a consistent 2-hour crisis response element to our community services with an appropriate skill mix of staff to support our current services across 12 hours (08:00 to 20:00) 7 days a week. We are also extending our services to provide nursing cover 24/7 in the community by developing an overnight element which will provide a much-needed service to our patients across Wiltshire. Overnight nursing shift times are 20:15 – 07:45.

Working for us will provide you with the opportunity to be part of an experienced team, working in a busy environment, supporting a registered nurse. You will be a strong team player, an excellent communicator who is adaptable, committed, organised and able to work under pressure. You will support the development, implementation, and maintenance of high-quality safe and sustainable clinical and operational leadership under the direction of the team managers and clinical lead.

The work will be highly rewarding, the successful candidate will support effective nursing interventions in patients’ homes and other community settings, supporting the prevention of hospital admissions and working with the 2-hour crisis response service. The role is varied and interesting, providing support to many patients with complex needs with a timely response to unplanned referrals.

Main Responsibilities

1. To implement treatment by following care plan.
2. To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
3. To undertake the daily care and support of delegated patients.
4. Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
5. Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting.
6. Responsibility for the day-to-day care of patients including monitoring.
7. Support the taking of medication under the authorisation of the qualified practitioner.
8. To have a good knowledge of infection control, to reduce the risk of healthcare-acquired infections.
9. To enable patients to reach their optimum level of independence.
10. Support terminally ill patients and their carers/relatives.
11. To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
12. To ensure the needs and wishes of the individual are documented.
13. To work within a multi-professional team.
14. To undertake specific tasks as delegated.
15. To assist registered and junior staff, referring to registered staff when necessary.
16. To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory body’s guidelines.
17. To obtain consent for any treatment undertaken.

The Ideal Candidate

1. Good general education.
2. NVQ Level 3 in healthcare – or willingness to achieve.
3. Experience of working under distanced supervision.
4. Experience of supporting other workers.
5. Ability to work as a team member.
6. Delivering patient-focused care.
7. Ability to recognise own limitations.
8. Typing/data inputting skills.
9. Physical skills to manually handle patients and use appropriate lifting aids.
10. Undertake specific nursing/therapy skills.
11. Current full driving license and use of a car during working hours.
12. Willing to work in other areas of HCRG Care Group as and when required to do so.
13. Driven to achieve.
14. Self-motivated.
15. Good time management.
16. Food Hygiene certificate or willingness to undertake.
