Job description

You will ensure that the medicine management system for the practice is implemented and working well. This includes ensuring cost-effective prescribing, supporting colleagues and patients with medicine queries, and issuing prescriptions as an independent prescriber.

You will be guided by targets and requirements set by the NHS locally and nationally for our practice.

Main duties of the job
  • Review and action safety alerts from the MHRA or other bodies, for the practice
  • Carry out medication reviews for specific groups of patients, including those on high-risk medicines
  • Reconcile medicines from hospital discharges or specialist appointments, and for newly registered patients
  • Review and action any directives from BSOL ICB with regards to medicines management requests or requirements
  • Encourage the practice into prescribing in a cost-effective manner, including use of the BSOL Formulary
  • Be a reference point for colleagues for questions regarding medications
  • Perform audits on medicines topics as directed by the practice
  • Update colleagues on the latest guidance regarding medicines, including from e.g. NICE
  • Issue repeat medicines and reauthorise medicines as appropriate
  • Assist with compliance with national targets, including e.g. QOF

The Limes Medical Centre is looking for a clinical pharmacist that is comfortable working within a close team, to ensure our service provides care that is safe and effective. We are looking for a pharmacist that understands the needs and requirements of general practice medicines management, and is able to contribute to our desire for high quality care.

We are based across two sites, with the main site in Small Heath being the primary location. We are a practice that trains future GPs, and we are looking for innovative ways to ensure our service is sustainable whilst meeting the needs of our community. Having a clinical pharmacist join us is one step we are taking to help us deliver this.

Job responsibilities

The Limes Medical Centre wants a motivated clinical pharmacist who ideally has some experience of working in general practice previously. Your responsibilities encompass the full range of medicines management, including prescribing, reconciliation, optimisation, advice to colleagues, and issues related to clinical governance and safety.

You will be a key on-site presence for your colleagues and patients, delivering the following as part of your normal working schedule:

  • Medication Reviews: Conduct regular medication reviews for patients with chronic conditions to ensure optimal drug therapy. You will also perform more detailed reviews on patients that are prescribed higher risk medicines.
  • Patient Counselling: Provide advice directly to patients on the best approach to using their medications, including their appropriate treatment regimen, and advice on e.g. side effects.
  • Clinical Advice: Provide advice to colleagues that have queries on prescribing, appropriateness, required counselling, and possible alternatives to medication decisions.
  • Medication Audits: Conduct regular audits to ensure that the practice is compliant with national guidelines and local regulations regarding medication storage, prescription practices, and patient safety.
  • Regulatory Compliance: Ensure compliance with all pharmaceutical laws and regulations, including those related to controlled substances, medication safety, and record-keeping.
  • Prescription Management: Monitor prescription patterns and make suggestions for improvements in prescribing practices to enhance patient safety and cost-effectiveness.
  • Training: Provide training and guidance to other team members regarding pharmaceutical matters, ensuring everyone is informed about the safe use of medicines.
Person Specification
Experience
  • Previous experience working as a pharmacist in general practice.
Qualifications
  • A valid MPharm degree and registration with the General Pharmaceutical Council (GPhC).
