Calderdale Council is now looking for a Pathways Team Member / Personal Assistant for Care Leavers. Position Details:

This is a temporary ongoing long-term post covering maternity leave for at least 9 months. Office based, 37 hours a week, £16.48 – £20 an hour.

About Us:

Join our dynamic and dedicated Pathways Team, where we work passionately to support vulnerable and at-risk young people aged 17.6 to 25, including care leavers and Unaccompanied Asylum-Seeking Children. Our mission is to provide high-quality support services that empower young people, improve outcomes, and help them transition into independent living with creativity and flexibility.

Key Responsibilities:

  1. Manage a caseload of young people, acting as a Corporate Parent to provide advice, assistance, and support.
  2. Co-produce and review pathway plans to enable successful transitions to adulthood and independence.
  3. Actively contribute to the Team’s Participation & Enrichment offer, including age-appropriate group activities.
  4. Collaborate with internal and external services, local businesses, and higher education providers to develop opportunities in education, employment, and social enrichment for young people.
  5. Act as a thematic lead in a specific area, bringing your skills and knowledge to enhance the team’s work.
  6. Manage competing priorities while maintaining high standards and meeting statutory responsibilities.
  7. Work collaboratively in a multidisciplinary environment to deliver cohesive and supportive services.
  8. Maintain accurate and timely records using the CASS system to meet key performance indicators.
  9. Represent the service professionally, demonstrating a commitment to equality, diversity, and continuous improvement.
  10. Participate in the duty system and be willing to work flexibly, including evenings or outside normal office hours. You will also be expected to travel out of the borough to visit young people who are placed out of the city.

Person Specification:

  1. Relevant Level 3 qualification or above (e.g., Social Work).
  2. Case holding experience.
  3. Full, valid UK driving licence.
  4. Up-to-date knowledge of legislation and guidance related to children and young people.
  5. Proven ability to assess, create support plans, and manage risks effectively.
  6. Strong communication, organisational, and record-keeping skills with attention to detail.
  7. Knowledge of safeguarding requirements and ability to make sound judgments in risk assessment.
  8. Demonstrated commitment to promoting equality and diversity.
  9. Experience working with young people, particularly those at risk of exclusion, and collaborating with families and communities.
  10. Experience in systemic practice, partnership working, and managing caseloads independently.
