Community Mental Health Nurse East London NHS Foundation Trust

Across North-East London, we have been developing new ways of bringing together health and social care professionals with our Voluntary sector and local communities to support people with severe mental health problems, including complex emotional needs associated with a diagnosis of personality disorder and disordered eating.

Our transformation work aims for teams of clinical and non-clinical staff to work together to make a greater positive impact on the health and lives of groups and communities of people living with mental ill-health. Through providing more integrated care across mental and physical health services in inner London, we have been able to test new ways of working within our Neighborhood teams, to deliver improvements aligned to the implementation of the Community Mental Health Framework for Adults and Older Adults.

This is a permanent community nursing and care coordinator role within the diverse Bethnal Green and Globe Town Neighborhood Mental Health Team.

The post holder will:

  1. Promote the needs of service users with mental health needs and offer training and education whenever possible.
  2. In conjunction with the crisis pathway services, provide a safe and effective urgent community assessment and treatment response service.
  3. Undertake comprehensive assessments, including risk assessment, effectively and efficiently.
  4. Formulate and agree crisis safety care plans and support and advise relatives and caregivers.
  5. Act as care coordinator for service users.
  6. Proactively involve people in their care and include them in decision-making processes.
  7. Assess, plan, set goals with the service user, carers, and families; implement and evaluate care using psycho-social interventions and skills to meet individual needs.
  8. Be responsible for accurate recordkeeping in line with Trust and NMC policy and procedures.
  9. Plan and prioritize own workload.
  10. Administer and/or monitor medication as prescribed in keeping with policies and NMC requirements.
  11. Act as an assessor to student nurses on placement.
  12. Contribute to establishing and maintaining liaison with the members of the primary/secondary healthcare team and voluntary agencies to provide a comprehensive service to clients and their families.
  13. Participate in duty, organizing the work of the whole team as required and directed by the Operational Lead or Senior Mental Health Practitioners in accordance with Team protocols.
