Are you an experienced Band 6 nurse thinking of a career in Admission Avoidance and Intermediate care service? As a senior nurse, you will be responsible for providing highly skilled nursing care to patients who reside in the Borough of Tower Hamlets. You will be promoting the health of this population and reducing the effects of ill health, disability and maximising the individuals potential for independence. The primary focus of this role is around prevention and early intervention. This role will play a key part in delivering a community-centred, integrated approach to health and social care, aiming to improve the outcome of frail, older adults and reduce the need for hospital admissions.
Our team is part of a wider established integrated service. This will give you the opportunity to work closely with our Physiotherapists, Occupational Therapists, Care Navigators, Mental Health Nurse, Physician response team in Royal London, the London Ambulance service and Psychologist. Our service is also supported by a team of experienced administrators. You will also have the opportunity to work closely with the local General Practices, Tower Hamlets Social Service team and other statutory and non-statutory services to meet the health and social needs of this local population.
