Job description

This is an opportunity to join our team of dedicated professionals providing services to some 9,500 patients in the rural market town of Dursley, Gloucestershire. We are expanding our Pharmacy Team and our current Pharmacist would be happy to talk with anyone interested in the post. Please contact the recruiting lead to arrange this. Informal visits are also welcome.

The role is practice based, managing patients face to face, via the telephone and administrative work.

We are aiming to hold interviews on 1st May 2025.

Main duties of the job

This is an exciting opportunity to grow and develop our Pharmacy team as we recognise the potential of Clinical Pharmacists in general practice.

The role will include some direct patient contact including chronic disease management, clinical medication reviews, medicine optimisation but will also include administrative work including clinical audits, QOF, patient letters, discharge summaries and helping with queries.

We would like a Non-medical prescriber or a willingness to work towards this qualification.

Chronic Disease Management knowledge including Asthma, COPD, Diabetes, CKD, AF and Hypertension is desirable or an interest in training in these areas.

Medication Safety & Governance knowledge, using audits with Ardens and Systm1.

Job responsibilities

Clinical Medication Review

Undertake clinical medication reviews with patients and produce recommendations for nurses and/or GP with regard to prescribing and monitoring for Practice Patients, Care Home Patients to include MDT Case Conferences. These reviews may be face to face, telephone or by text.

Medicine information to practice staff and patients

Respond to medicine related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients/carers with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.

Management of Medicines at Discharge from Hospital

To reconcile medicines following discharge from hospitals, intermediate care and into care homes, working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

Identification of patients at risk of harm from medicines through computer searches. This might include risks that are patient related, medicine related, or both.

Medicines Quality Improvement

Undertake simple audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team.

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Implementation of Local/National Guidelines and Formulary Recommendations

Agree and review Prescribing Formularies and Protocols and monitor compliance levels.

Support the delivery of QOF, incentive schemes, QIPP and other quality or cost-effectiveness initiatives.

Care Quality Commission

Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

Public Health

To support public health campaigns. To provide specialist knowledge on all public health programmes available to the general public.

Working Relationships

Recognise the roles of other colleagues within the organisation and their role to patient care, whilst working as part of the surgery.

Should have excellent communication skills.

Actively work toward developing and maintaining effective working relationships both within and outside the practice and locality.

See patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma). Review the ongoing medicine plan for these patients, ensuring they get the best use of their medicines (i.e. medicines optimisation) and make appropriate recommendations to clinicians for medicine improvement.

The NHS changes and as it does members of the team need to change so a successful candidate will need to be open to a changing job role.

Person Specification
Qualifications
  • Registered member of General Pharmaceutical Council (GPhC)
  • Evidence of commitment to continue professional development to maintain advanced knowledge.
  • GPhC Independent Prescribing Qualification (or working towards attainment)
  • Member of the Primary Care Pharmacy Association (PCPA)
  • Member of the Royal Pharmaceutical Society
  • Undertaken, completed or can demonstrate exemption from CPPE's Primary Care Pharmacy Education Programme
Experience
  • An appreciation of the nature of Primary Care and General Practice.
  • Minimum of 3-years post registration experience.
  • An understanding of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
  • Excellent communication skills, verbal and written
  • Good at problem solving and meeting deadlines.
  • Produce informative on time.
  • Adaptable.
  • Self-motivated
  • Able to meet deadlines and work under pressure with interruptions.
  • Experience in primary care working within a GP surgery.
  • Knowledge of SystmOne system.
  • Knowledge of Ardens system.
  • Basic Life Support training.
