Independence Healthcare, a Modivcare Personal Care Service, is looking for a RN Field Supervisor responsible for ensuring quality and safe delivery of home health care services such as supervising Caregivers / Certified Home Health Aides in a patient's home, evaluating the patient's plan of care and assessing their physical and psychosocial needs.

• Schedule: M-F 8:30 AM - 5:00 PM
• Visiting Client Territory: Brighton and surrounding areas
• Office: 18R Shepard Street, Brighton, MA 02135
• Pay Range: $43 - $45 / hourly rate range

You will

• Perform on-site supervisory and teaching visits to evaluate caregivers
• Provide clinical orientation to caregivers on assigned home care cases
• Participate in the training of new nurses and in-services for field staff
• Perform initial nursing assessments
• Counsel patients/caregivers in meeting nursing and related needs
• Assign and coordinate services provided
• Assist physicians in developing a plan of treatment/care and make necessary revisions based upon the patient's condition/needs
• Prepare clinical progress notes ensuring any changes in the patient's condition are reported
• Monitor clinical records for complete and accurate documentation
