Main duties of the job
  • Lead the clinical management of a designated group of service users.
  • Allocate work to junior colleagues, reviewing progress and agreed outcomes.
  • Co-ordinate and monitor the delivery of person-centred healthcare to a designated group of service users, using complex clinical decision making where appropriate.
  • Work as a member of the multi-disciplinary team in assessing and planning care and responding to changes in service users' health.

  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • Ensure accurate records are kept in accordance with CHS policy and NMC guidelines.
  • Undertake regular mandatory training.
  • Full UK driving licence and own vehicle.

*Depending on the Trust, you may require variable additional certification and skills.
