An exciting opportunity has arisen for a qualified ACP to join our Gastroenterology service. We are seeking a committed, enthusiastic, and compassionate individual to join our existing team at Great Ormond Street Hospital (GOSH), which is one of the leading specialist departments in the UK and sees around 5000 children and young people each year. As a member of a team comprised of staff from all professional groups, you will be working with babies, children, and young people to support the highly specialised field of Inflammatory Bowel Disease/ Mucosal immunology.

The advanced clinical practitioner will have met the required academic and practice capabilities required to practice at the level of Advanced Practice. The below scope and capabilities of this role is based on the key capabilities described in the Multi-Professional Framework for Advanced Practice (HEE 2017)
- To be a proactive, dynamic clinical leader within the healthcare team
- To provide direct expert advanced clinical care through health assessment, initiation of treatment, formulation and alteration of treatment and monitoring of care programmes
- To facilitate the development of evidence based clinical practice through improvement, research and audit
- Participate in the education of the multidisciplinary team and trainee ACPs.
- To evaluate through quality assurance initiatives and assessment of clinical outcomes
- To be an expert clinical resource for other healthcare practitioners

Detailed job description and main responsibilities
Main duties and responsibilities:
Clinical
- To be an expert practitioner and role model in the field of Gastroenterology healthcare and ensuring the delivery of high standards of patient care.
- To ensure the 6Cs and professional standards are embedded within clinical practice.
- To ensure clinical practice reflects professional and national guidance and practice is delivered in line with the scope of practice and trust policies and guidelines.
- To provide direct clinical care through health assessments and initiation of treatment for children attending for procedures/treatments within ACP’s authorised scope of practice.
- To be responsible for the clinical management of children / young people, utilising clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes where required.
- To respond to clinical emergencies, utilising clinical judgement and decision making to initiate appropriate interventions and treatments for patients.
- To counsel and support the child, young person and families through the treatment process, utilise advanced decision-making skills and demonstrate understanding of when to refer on.
- To initiate drug therapy/ medication as appropriate, practicing as an independent prescriber for patients within area of competence.
- To promote family centred care, maintaining a safe, friendly and welcoming ward or department environment for children, families, visitors and staff.
- Act as an advocate for the child/young person and family.
- Utilising excellent communication skills support effective communication between the MDT and the patient / family, to support the delivery of a patient focused service and to facilitate effective co-ordination of care.
- To actively contribute to the decision-making processes regarding care and treatment for the patient, in particular with the senior registrar / consultant with overall responsibility for the patient.
- To make and receive referrals to and from the wider MDT.
- To support the development of clinical protocols and care pathways enabling the advanced clinical practitioner to assess and treat children within these agreed protocols and pathways.
- To be responsible for reviewing and updating clinical practices and guidelines within the area of practice, ensuring these reflect current knowledge, research and peer reviewed best practice.
- To provide up-to-date information for families, staff and shared care centres in conjunction with the multidisciplinary team.
- To utilise the role of the ACP to improve and maintain continuity of care for patients and their families.
- To monitor and evaluate clinical outcomes involving the patient and carers where possible.
- To develop appropriate advanced clinical and technological skills in response to the changing needs of healthcare.
- To take every opportunity to promote the health of children, young people, and their families and initiate health education / promotion strategies as appropriate.
- To develop and promote the use of ACP led performance indicators and outcome measures.
Leadership / Management
- To be a proactive clinical leader, demonstrating effective excellent leadership skills.
- To promote best practice, utilising local and regional benchmarking groups for the generation and promotion of standards and clinical guidelines.
- To support transformation / improvement work streams as appropriate.
- To act as a resource, providing clinical leadership and professional advice to the multidisciplinary team, within and outside the organisation.
- To liaise effectively with other clinical staff to ensure the most efficient and effective use of resources.
- To actively contribute to the management of risk, working closely with divisional team members and the risk management team.
- To manage clinical incidents and complaints and support the implementation of subsequent learning where appropriate.
