Detailed job description and main responsibilities

We are looking to recruit motivated, caring, compassionate and experienced Community Rehab Support Workers to join this new and exciting service within the South of Nottingham, covering the localities of Rushcliffe, Broxtowe and Gedling, working as an accountable and autonomous practitioner alongside the integrated care teams and acute Trusts to support discharge and prevent hospital admission.

You will work in a large and supportive multi-disciplinary clinical team with a dedicated focus of excellent patient care and innovation. Providing navigation, urgent response assessments and facilitation of discharge from the acute provider, focusing on the hospital avoidance agenda, discharge to assess and 2-hour/2-day guidelines and promoting flow through the system.

You will be involved in liaising and coordinate services with other health and social care providers to enable recovery from illness, promote health and wellbeing and promote self-care, fostering collaborative partnerships with local hospitals, EMAS, NHS111, GP's and our community nursing and therapy colleagues, along with Local Authorities and private providers.

As a Community Support Worker, you will:

  1. Assist in the provision of rehab services within the Urgent Community Response Team by implementing an agreed programme, set by qualified therapists and nurses, working at times without direct supervision in patients' own homes, residential homes or nursing homes.
  2. Provide rehabilitation with the aim of improving function for people through individual treatment plans and education.
  3. Provide community care.
  4. Undertake equipment follow up visits.
  5. Work collaboratively with other services including community therapy and the virtual ward to support patients to remain safe and independent at home.
  6. Collect data that will support the effectiveness of clinical interventions.
  7. Respond flexibly to new service developments and changes in working patterns, including hours of working.

You must have a full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Essential criteria

  • A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Experience

Essential criteria

  • Previous caring experience in a formal care setting.
  • Previous experience of working with patients with complex needs.

Desirable criteria

  • Experience in a rehabilitation role.
  • Previous wound care / clinical skills.
  • Previous experience of working in a community setting.
  • Experience of working in an acute setting.

Knowledge

Essential criteria

  • Communicate with patients and carers who may have cognitive impairment. To deal with challenging family situations, or circumstances including, care of the dying.
  • Demonstrate the ability to communicate and support individuals with life limiting conditions.
  • Knowledge of pressure area care.
  • Experience of working with manual handling equipment.

Desirable criteria

  • Knowledge of UCR national mandate and guidance.
  • Safeguarding.

Skills

Essential criteria

  • Excellent verbal /written communication and interpersonal skills.
  • Ability to work on own initiative and under pressure. Good time management skills.
  • Ability to work both independently and as an effective team player.
  • Ability to work flexibly.
  • Experience of undertaking physical observations.

Desirable criteria

  • Medication administration.

Qualifications

Essential criteria

  • NVQ Level 3 qualification (or equivalent) in a relevant health & social care area.

Desirable criteria

  • Additional learning including pressure area management, physical observations, manual handling etc.
