Job Title: Community Psychiatric Nurse

Trust: Livewell Southwest

Location: Plymouth

Shift Patterns: 9am to 5pm

Whilst not a formal part of the NHS, Livewell Southwest CIC is a not-for-profit organisation which works alongside colleagues from local NHS hospitals and services to deliver expert care. What you can add:

  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their well-being.
  • Signposting to appropriate care providers (including primary and tertiary care).
  • Raising concerns to senior staff for support and management.
  • Completing documentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology, and admin.

The role will involve, but not be limited to you performing the above.

What you'll need:

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • Full UK driving licence and own vehicle.

*Depending on the Trust, you may require variable additional certification and skills.
