The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city.

PROGRAM OVERVIEW

The NYC Department of Health and Mental Hygiene (DOHMH) seeks two Community Health Workers to join the Office of Faith-Based Initiatives (OFBI) within the Bureau of Health Equity Capacity Building. This role is part of the Center for Health Equity & Community Wellness (CHECW), dedicated to addressing health disparities and promoting health equity across New York City communities.

POSITION OVERVIEW

The Division of Center for Health Equity & Community Wellness (CHECW) uses a racial and social justice approach to eliminate health inequities for those who are most marginalized in New York City.

RESPONSIBILITIES

• Community Engagement & Outreach:
  • Conduct community outreach to connect with faith-based organizations, community members, and stakeholders.
  • Support the planning and execution of community events and health education programs.
  • Assist partners at program events, ensuring smooth operations and effective engagement.
  • Serve as a liaison between the community, faith organizations, and health services to address health inequities.
• Health Advocacy & Support:
  • Build relationships with community members, faith leaders, and local organizations to promote health initiatives.
  • Provide health education and social support to community members.
  • Assist in collecting health-related data and reporting findings to inform community health strategies.
• Administrative & Data Management:
  • Perform administrative tasks, including data entry into the Public Health Partner Connect (PHPC) and other preferred systems.
  • Support project management efforts, ensuring timely completion of assigned tasks.
  • Maintain accurate records of activities, events, and outreach efforts.
• Other Duties:
  • Participate in training and capacity-building activities to stay current on health equity and community wellness strategies.
  • Be flexible in assisting with additional duties as assigned to support the OFBI’s mission.

QUALIFICATIONS

• High School Diploma with 3+ years of relevant experience, or an Associate’s Degree with 2+ years of relevant experience. A Bachelor’s degree in a related field is a plus.
• Experience working with community-based organizations, faith communities, or public health initiatives.
• Bilingual proficiency (especially in Spanish or Haitian Creole) is highly preferred.
• Strong communication, relationship-building, and interpersonal skills.
• Ability to engage with diverse communities using a culturally sensitive approach.
• Strong organizational and project management skills with attention to detail.
• Advanced proficiency in Microsoft Office Suite (Excel, PowerPoint, Word).
• Ability to work independently, manage multiple projects, and meet deadlines.
• Client-focused, empathetic, and committed to health equity.
• Willingness to work evenings and weekends as needed to support community events.
• Ability to travel across all five boroughs of NYC using public transportation.
• Must be comfortable working in diverse community settings, including religious institutions and other non-traditional spaces.

SALARY AND BENEFITS

• Annual Salary range is $60,000 to $65,000

ADDITIONAL INFORMATION

There is potential for this position to transition to DOHMH and therefore candidates must meet DOHMH eligibility requirement including NYC residency.
