CRITICAL OUTPUTS

1. Assess patients' psychosocial needs and provide emotional support.
2. Advocate for patient rights and coordinate resources for optimal recovery.
3. Act as a liaison between patients, families, and healthcare teams to ensure clear communication.
4. Develop individualized discharge plans, including home care and community resource coordination.
5. Provide counselling and education on coping strategies, healthcare options, and lifestyle adjustments.
6. Work collaboratively with interdisciplinary teams to implement holistic care plans.
7. Address urgent psychosocial concerns such as financial hardship, housing insecurity, and mental health crises.
8. Maintain accurate patient records and ensure compliance with healthcare regulations.

REQUIREMENTS

1. Education : Bachelor of Social Work (BSW) degree & SACSSP registration.
2. Experience : 3–5 years as a Social Worker, preferably in healthcare or physical rehabilitation.

4. Code 8 driver’s license and own reliable transport.
5. Ability to work in a pressurized environment with on-call and overtime requirements.
6. Strong interpersonal, communication, and ethical decision-making skills.
7. Experience in interdisciplinary collaboration.
