A vacancy exists for an Occupational Health Nursing Wellness Case Manager, based at the Life Occupational Health clinic South 32 Northern Cape Hotazel. The successful candidate will report to a Unit Manager. The Occupational Health Case Manager will be responsible for managing site Occupational Health programs, EMS teams, and case management, rendering a quality service that is effective and efficient, meeting and exceeding the Client’s expectations and requirements.
Critical Outputs
To assist the Unit Manager and client with the management of healthcare programs related to Primary Healthcare, Chronic Illness, HIV, TB, Wellness, and Case management.
- In consultation with the client and other suppliers, compile and implement an annual health awareness plan and programme. Establish a year planner for a Health program. Assist with the implementation of wellness days and health awareness and education drives.
- Drive continuous quality programs and ensure a high quality of remote incident and case management, driving customer satisfaction and client retention.
- Comply with all relevant administration related duties and functions, including completion of registers.
- Comply with prescribed professional and quality standards of both Life Health Solutions and the Client.
- Assist with compliance to quality programs and conducting of internal quality, client, and external audits.
- Work effectively within a team.
- Ensure total client satisfaction through continuous improvement and added value.
- Ensure compliance with ISO 9001:2008 and OHSAS 18001:2007 standards, including internal and external audits (Life Employee Solutions as well as client audits).
- Provide expert professional advice to employees and ensure the maintenance of clinical excellence.
- Prevent ill-health, promote good health, and well-being amongst employees, including testing, monitoring, referral, and follow-up.
- Conduct examinations, tests, and treatment where applicable concurrently with the Occupational Health Team.
- Establish fitness to work, with feedback to the line managers or referral to the on-site OMP, should a problem be identified.
- Work effectively within a pre-determined budget, comply with stock control measures and requirements, including the ordering of stock.
- Provide case management through assessment and monitoring of chronic conditions, developing individual follow-up plans, and ensuring employee compliance to treatment regimens.
- The Case Manager will be accountable for the operational concerns regarding all client injury on duties, administration thereof, and reporting.
- Remotely monitor patient care during all stages of pre-hospital, in-hospital, and out-of-hospital specialist treatment.
- Liaise with members of the multidisciplinary team, employees’ private doctors, local health authorities, and hospitals.
- Function as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose, and track the conditions of employees.
- Keep up to date with innovative methods and practices to deliver exceptional primary care, Wellness, and case management services to meet the needs of the client.
- Assist with compiling information and data for feedback on identified health problems and trends, including progress regarding the on-site Chronic Illness Monitoring, TB, HIV, and Wellness program.
- Assist with compiling accurate written and statistical feedback reports at specified intervals, including DOH and DMR reports as required.
- Assist with updating relevant registers.
Requirements
The following critical qualifications and skills are required:
- Nursing Diploma or Degree, qualified as a Registered Nurse with a Diploma in Occupational Health Nursing (Must appear on current SANC receipt).
- A relevant qualification in Wellness Management and hold a certificate in HIV testing and counselling, treatment of TB and STIs.
- Certificate in Audiometry, Spirometry, Vision, Multidrug test, HIV counselling, and dispensing.
- Registered as an audiometrist at SASOHN.
- Computer literacy in Microsoft Office (Medical database experience advantageous).
- Two to three years’ experience in the provision of Primary Health and Wellness services, in an Occupational Health and Mining Clinic.
- Certificate in Dispensing.
- Registered with SANC.
- Experience in management of multidisciplinary teams, administration, employee relations, and communications.
- Continuous focus on a high standard of service delivery, client communication, satisfaction, and teamwork.
- Candidates who are results-oriented, focused, well-organised, with good communication skills and excellent administrative, coordination, and management skills, including compiling Client feedback reports (includes statistical feedback), will be most suitable for this position.
- Own transport is essential, with a Driver’s license Code 8 and own accommodation.
- Experience in COID act reporting and administration.
- Candidates with the ability to deliver quality outputs and reduce costs will be most suitable for this position.
