Job description

A vacancy exists for a Renal Care Coordinator based at Life Peglerae Hospital, reporting to the Patient Services Manager.

The responsibility of the Renal Care Coordinator will be to facilitate the enhancement of more holistic disease management for patients by seamlessly integrating currently disjointed aspects of the renal journey. This role aims to improve patient care by supporting the Renal Unit Manager and the Multidisciplinary Team on care activities.

Critical Outputs

Financial Risk Management

  1. Identifying deviations from the clinical pathway.
  2. Manage the financial risk related to the clinical pathway including clinical pre-assessment, authorisation, and re-authorisation (including clinical coding).
  3. Support the Bill Auditor to close out any funding disputes related to clinical pathway products.
  4. Monitor and report on the cost of the event.
  5. Analyse trends to identify deviations and influence the responsible persons through presentations and discussion.

Clinical Risk Management

  1. Clinical pre-assessment following initial diagnosis.
  2. Acting as the primary point of contact for patients from after initial diagnosis until the patient has sufficient knowledge and resources to self-direct care.
  3. Supporting the patient and family with education in respect of all care needs as they journey through the Life Healthcare system.

Co-ordination/Facilitation Role

  1. Build awareness of the Life Care Coordinator role with internal and external service providers.
  2. Receive criteria-based referrals from relevant care providers and respond within 8 working hours.
  3. Track patients referred with a diagnosed health pathway to ensure all diagnostics and follow-up appointments are booked to facilitate patient transition to the next step of care program.
  4. Track patients' test results to determine what further action is required, liaising with Dr and/or members of the multidisciplinary team (MDT).
  5. Provide administrative support and facilitate MDT meetings including preparing patient notes, images, etc., and organizing and recording the content of meetings.
  6. Update each patient record with the outcome of MDT meetings and communicate with relevant healthcare partners as required.
  7. Take responsibility for all outcomes identified for the Life Care Coordinator at MDT meetings and action them in a timely manner liaising with internal and external healthcare partners.
  8. Assess patient needs for support services and suggest referrals.
  9. Monitor, coordinate, and track the care of a patient on clinical care pathways to improve health, treatment, or end-of-life outcomes.
  10. Assess the care and services required by the patient and make the necessary referrals to the multi-disciplinary team/support service partners.
  11. Post-discharge follow-up and monitoring.
  12. Complete data reports and patient records in relation to interventions.
  13. Educate patients by providing information on available care options to empower the patient to make informed decisions and participate in their plan of care/goal setting.
  14. Provide emotional support to patients and families to navigate the journey through the relevant care pathway.
  15. Prioritize own workload and develop systems to improve the effectiveness of the role.

Requirements

  1. Grade 12 - Matric.
  2. Nursing Qualification (SANC), minimum requirement as a Renal registered nurse with at least 3-5 years’ experience or HPCSA registration appropriate to relevant Medical Qualification such as a Renal Technologist and/or Clinical Associates.
  3. Knowledge of hospital patient services and billing processes.
  4. Knowledge and understanding of CPT/CCSA & ICD coding.
  5. Knowledge of funder contracts, scheme rules, exclusions, and benefits.
  6. Knowledge or clear understanding of hospital industry and practices.
  7. Computer proficiency.
