Job Title: Clinical Pharmacist

Location: Botshelong-Empilweni Hospital

Reports to: Responsible Pharmacist

Job Summary
Clinix is seeking a dedicated and motivated Clinical Pharmacist to be responsible for leading, managing, coordinating, and monitoring clinical pharmacy services and driving pharmaceutical improvement within the hospital. The clinical pharmacist will conduct daily ward rounds, review patient medication profiles across the hospital division, and contribute to improving patient outcomes.

Minimum Qualifications And Experience

1. Pharm Degree and a post graduate qualification in Clinical Pharmacy
2. Active registration with the South African Pharmacy Council
3. Five (5) years’ experience as a pharmacist (post internship), of which two (2) years must be in Clinical Pharmacy inclusive of Antibiotic Stewardship, Pharmacovigilance, and general Rationale Medicine Use
4. Computer Proficiency in Microsoft 365 applications like Excel and PowerPoint
5. Excellent communication (written and verbal) and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams
6. Successful training and understanding of the Blue Bird programme and ability to work on SAP will be additional benefits.

Minimum Job Requirements

1. Ensure the provision of customer-focused, professional clinical pharmacy services by conducting daily ward rounds to perform comprehensive medication reviews for patients, monitoring responses to medications, and making necessary interventions to ensure optimal therapeutic outcomes
2. Ensure medication safety and timely resolution of medication-related challenges
3. Elevate antimicrobial stewardship performance as a key focus area
4. Monitor microorganisms and collect and analyze Antimicrobial Stewardship data to identify inappropriate utilization of antimicrobials
5. Drive interventions to optimize drug utilization and pharmaceutical care
6. Active participation in internal stakeholder meetings, including presentation of progress and gaps
7. Professional collaboration by enhancing cooperative and influential working relationships with doctors, microbiologists, nursing staff, infection prevention control, pharmacists, and other key functional stakeholders to achieve key outputs
8. Provide evidence-based drug information and recommendations on medication therapy management, drug utilization, and formulary management
9. Participate actively in interdisciplinary discussions regarding patient care and medication management
10. Maintain accurate and detailed patient records, documenting interventions, recommendations, and outcomes of medication reviews
11. Role in development and implementation of drug administration protocols to proactively manage risk
12. Medication adverse event reporting and interventions
13. Provide workplace-based support and mentoring to pharmacists to develop skills, capacity, and professional competency within the pharmacy
14. Prepare and present reports to the Clinix Clinical Risk Committee
15. Actively engage in continuous education and training to enhance clinical knowledge and skills and encourage pharmacists in operational research
