Role Description

This is a full-time on-site role for a Chronic Care Nurse at Star Nurse Educator located in the City of Cape Town and Durban. The Chronic Care Nurse will be responsible for chronic care management, disease management, nursing tasks, and providing specialized care for individuals with chronic conditions.

Requirements

1. Chronic Care Management and Chronic Care skills
2. Enrolled Nurse
3. Enrolled Nurse Assistant
4. Pharmacy Assistant
5. Nursing and Disease Management skills
6. Strong communication and interpersonal skills
7. Ability to work in a fast-paced healthcare environment
8. Current nursing license
9. Previous experience in healthcare
