Overview

We are looking for a Remote RN Care Manager AIM (Advanced Illness Management) to join our team to cover our Michigan palliative care branches. This position will directly report to the Director of Care Management AIM and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range – $80-83K/year

About You

• 2 years of prior skilled Registered Nurse experience
• Active acute unencumbered state RN license
• Must be willing and able to be licensed as an RN in all states applicable to role - Michigan Licensure
• Ability and interest to provide direct patient care in homes and facilities
• Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team
