Job description

PPO Serve is looking for a Care Coordinator to assist patients and clinicians in the projects we support. The Care Coordinator would actively identify and onboard complex patients, facilitating the ongoing care of these patients in an appropriate care setting, in collaboration with the consortium multidisciplinary team, patient, family, and local community services. This requires building and maintaining excellent relationships with all levels of healthcare providers and patients. The Care Coordinator is responsible for scheduling, organizing, and managing all aspects of a patient’s healthcare maintenance and treatment. Duties include helping patients complete paperwork, communicating with the healthcare team about a patient’s treatment plan, and educating patients about resources and options for managing their health.

YOU MUST BE ABLE TO:

You must be able to challenge the status quo if it is not working and have the courage to fix it. In addition, you should have a broad range of skills, work hard and smart. You can find answers fast and enjoy the process of building something new. You must be comfortable working with all levels of healthcare professionals from social workers to nurses to GPs to specialists. You must be comfortable with Word, Excel, and learning new systems.

WHAT WE NEED YOU TO DO:

  1. Patient management:
  • Perform health assessments on patients to identify individual patient risk factors.
  • Propose interventions as pre-defined to influence individual patient risk.
  • Actively participate in the management of complex patients by building good relationships with the patient and their family.
  • Monitor patient progress through ongoing patient and clinician interactions.
  • Promote behaviour change to ensure greater treatment compliance and condition management.
  • Coordinate the integration of care including social services.
  • Assist in navigating additional benefits.
  • Arbitrate and resolve conflict and interface with problems that arise in patient care.
  • Coordination of Care:
    • Regular liaising and communication with the care team (including clinicians, hospital, family members, counselors, and admin staff).
    • Assistance in the documentation of clinical information.
    • Maintain relationships between external treating providers, the consortium’s multidisciplinary team, and the management team.
    • Build teams in the healthcare industry – being able to inspire teamwork at all levels to ensure optimal outcomes for the patient and the team.
    • Understand the team’s operational processes for improvement.
  • Clinical process development:
    • Understanding clinical processes and engaging with clinical data to understand the issues.
    • Provide clinical input into a broad range of clinical developmental work including the development of templates and processes to improve the working of the team and the patient outcomes they produce.
    • Actively contribute to improvements to ensure the success of the bigger team.

    Desired Experience & Qualification:

    • Clinical Associate or Registered Nurse: Bachelor of Clinical Medical Practice/ Degree in Nursing.
    • 5 years industry experience with a minimum of 2 or more of the following:
      • 3 or more years of clinical experience.
      • Care Management or similar experience.
      • Multidisciplinary team experience.
    • Valid HPCSA registration.
