Job description

A vacancy exists for a Care Coordinator based at Life Kingsbury Hospital reporting to the Patient Services Manager, Elsabe Redelinghuys. The responsibility of the Acute Care Coordinator will be to facilitate the enhancement of more holistic disease management for patients by seamlessly integrating currently disjointed aspects of orthopaedic, stroke, and other surgical journeys. Improving patient care by supporting the Unit Manager and the Multidisciplinary Team on care activities.

Critical Outputs

  1. Patient management:
    1. Perform health assessments on patients to identify individual patient risk factors.
    2. Propose interventions as pre-defined to influence individual patient risk.
    3. Actively participate in the management of complex patients by building good relationships with the patient and their family.
    4. Monitor patient progress through ongoing patient and clinician interactions.
    5. Promote behaviour change to ensure greater treatment compliance and condition management.
    6. Coordinate the integration of care including social services.
    7. Assist in navigating additional benefits.
    8. Arbitrate and resolve conflict and interface with problems that arise in patient care.
  2. Financial Risk Management:
    1. Identifying deviations from the clinical pathway.
    2. Manage the financial risk related to the clinical pathway including clinical pre-assessment, authorisation and re-authorisation (including clinical coding).
    3. Support the Bill Auditor to close out any funding disputes related to clinical pathway products.
    4. Monitor and report on the cost of the event.
    5. Analyse trends to identify deviations and influence the responsible persons through presentations and discussion.
  3. Coordination of Care:
    1. Regular liaising and communication with the care team (including clinicians, hospital, family members, councillors, and admin staff).
    2. Assistance in the documentation of clinical information.
    3. Maintain relationships between external treating providers, the consortium’s multidisciplinary team, and the management team.
    4. Build teams in the healthcare industry – being able to inspire teamwork at all levels to ensure optimal outcomes for the patient and the team.
    5. Understand the team’s operational processes for improvement.
    6. Build awareness of Life Care Coordinator role with internal & external service providers.
    7. Receives criteria-based referrals from relevant care providers and responds within 8 working hours.
    8. Tracks patients referred with a diagnosed health pathway to ensure all diagnostics and follow-up appointments are booked to facilitate patient transition to next step of care program.
    9. Tracks patients test results to determine what further action is required, liaising with Dr and/or members of the multidisciplinary team (MDT).
    10. Provides administrative support and facilitates MDT meetings including preparing patient notes, images etc. and organizing and recording the content of meetings.
    11. Updates each patient record with outcome of MDT meetings and communicates with relevant healthcare partners as required.
    12. Takes responsibility for all outcomes identified for Life Care Coordinator at MDT meetings and actions in a timely manner liaising with internal and external healthcare partners.
    13. Assesses patient needs for support services and suggest referrals.
    14. Monitoring, coordinating and tracking the care of a patient on clinical care pathways to improve health, treatment or end of life outcomes.
    15. Assessing the care and services required by the patient and for making the necessary referrals to the multi-disciplinary team/support service partners.
    16. Post-discharge follow-up and monitoring.
    17. Completes data reports and patient records in relation to interventions.
    18. Educates patients by providing information on available care options to empower the patient to make informed decisions and participate in their plan of care/goal setting.
    19. Provides emotional support to patients and families to navigate the journey through the relevant care pathway.
    20. Prioritizes own workload and develops systems to improve effectiveness of post.
    21. Clinical process development:
    22. Understanding clinical process and engage with clinical data to understand the issues.
    23. Provide clinical input into broad range of clinical developmental work including development of templates and processes to improve the working of the team and the patient outcomes they produce.
    24. Actively contribute to improvements to ensure success of the bigger team.

Requirements

  • Knowledge of hospital patient services and billing processes.
  • Knowledge and understanding of CPT/CCSA & ICD coding advantage.
  • Knowledge of funder contracts, scheme rules, exclusions and benefits advantage.
  • Knowledge or clear understanding of hospital industry and practices.
  • Computer proficiency.
