About This Opportunity: CareATC is currently looking for a Mental Health Case Manager to work closely with Mental Health Professionals, Medical Providers, and Health Center Staff to ensure seamless access to mental health services for patients by coordinating scheduling, referrals, and follow-ups. This position serves as a bridge between patients and mental health providers, assisting with initial registration, appointment coordination (virtual or in-person), and ongoing patient support. The Case Manager is an integral part of the mental health team, providing education, advocacy, and crisis navigation when necessary.
This is a full-time, 40-hour-per-week, benefit-eligible, remote opportunity.
The role of the Mental Health Professional:
- Facilitate the scheduling of mental health appointments, both virtual and in-person.
- Assist with the initial patient registration process and gather necessary documentation.
- Coordinate referrals for specialized mental health services as needed.
- Provide education on available mental health services, treatment options, and resources.
- Monitor patient progress and follow up to ensure continuity of care.
- Work closely with mental health professionals, primary care providers, and other health center staff to ensure a holistic approach to patient care.
- Advocate for patient needs and assist with navigating barriers to care.
- Assist in identifying patients in crisis and connecting them with appropriate resources.
- Support patients in accessing urgent mental health services when needed.
- Excellent organizational, communication, adaptability, and time management skills are essential.
- Assist with tracking engagement activities and events that promote mental health awareness and education.
- Assist with wellness initiatives and health programs. Assist with coordination of workshops and/or educational seminars. Participation in local events if applicable.
- Community Resources: Connect patients to various community programs, help them find additional resources, and assist in decreasing the patient’s social stressors.
- Interdisciplinary Approach: Collaborate with the clinical care team to encourage utilization of resources that will improve the patient’s health and wellness. The Mental Health Case Manager will assist with the integration of the Total Health Solution initiatives, Mental Health Professionals, and Health Center Staff.
- Must be able to work independently, exhibit critical thinking, and prioritize tasks to meet commitments that are aligned with department goals.
- Strategic thinker who can analyze problems, prioritize solutions, and manage multiple complex situations while collaborating with team members as required.
- Strong telephonic skills are a must. Identify Patient's Social Determinants of Health needs within the population they are serving and address barriers by referring patients to community resources, RX navigation, or helping patients to understand insurance benefits.

Minimum Qualifications:
- B.S.W (Bachelor of Social Work) degree from a school of social work fully accredited by the Council on Social Work Education (CSWE) or a related program is required.
- Minimum of 12 months of experience, in the last 24 months, within case management, mental health services, or healthcare coordination preferred.
- Minimum of at least 12 months of direct hands-on experience in the last 24 months with computer operations, basic word processing, and data entry.
- Proficiency in electronic medical records (EMR) and scheduling processes.
- Recent experience (within the past 3 years) providing integrated behavioral case management services within a primary care setting, preferred.
