Care Coordinator is responsible to work in collaboration with multi-disciplinary health care teams and community service providers to provide coordination and continuity of patient care across the healthcare continuum; to support patients and their family members to transit care from hospital to home and in the community.
Responsibilities and Duties:
Participate in discharge planning and identify patients requiring post-discharge care transition support.
Plan and carry out post-discharge support and follow up via phone call and/or home visit to ensure patients and caregivers can cope at home and the discharge care plan is implemented.
Assist patients and their families to streamline and coordinate the range of community services available.
Implement timely and appropriate care coordination and evaluate the outcome accordingly.
Act as liaison, collaborating with patients, families, and multidisciplinary teams to achieve the best outcomes for patients.
Monitor patients' general medical condition during home visits and report to clinicians if necessary.
Promote positive changes in patients' lifestyles in the community.
Help patients and their families plan for and improve end-of-life care.
Participate in hospital activities that contribute towards improving the quality of patient care, such as research, quality, or innovative projects that enhance patient care.
Support data collection and preparation of reports and statistics for outcome tracking.
Job Requirements:
Possess a diploma/degree or equivalent professional qualifications in Nursing.
At least 3-5 years of experience in nursing or a related field in acute and/or community settings in Singapore.
Understanding of the Singapore healthcare system, community services, and resources for patients post-discharge.
Experience in care coordination or case management is highly valued.
A critical thinker, self-motivated, and able to work collaboratively with a multi-disciplined team.
Excellent interpersonal and communication skills.
Able to speak local languages and dialects will be an advantage.
Familiar with MS Word, Excel, and PowerPoint.
