IDHealth, an initiative of Happee Hearts Movement is a community-based, integrated health team dedicated to serving adults with intellectual disability and their caregivers.
People with intellectual disability have poorer health and lower access to and use of healthcare services than the general population. IDHealth’s goal is to reduce the barriers to health and healthcare utilization for this population by providing a health service that is accessible to and appropriate for people with intellectual disability. In addition, we aim to increase the capability of Singapore’s health and social service sectors to serve this population.

Roles and Responsibilities:
The nurse will work within an interdisciplinary team to support persons with intellectual disability (PWID) and their families in achieving positive health and wellbeing.
Patient
- Comprehensive assessment of the nursing issues facing PWID and their caregivers within the biopsychosocial-environmental context.
- Developing an intervention plan for the issues and challenges identified for both PWID and their caregivers.
- Implementing interventions based on the plan derived.
- Reviewing and monitoring the interventions and plans.
- Supporting behaviours of concern and communication needs in PWID together with their caregivers and other service partners.
- Supporting the PWID and their family in future care planning, including mental capacity, legal and financial matters.
- Supporting the PWID and their caregivers in advance care planning and end of life issues together with other service partners.
Team
- Working closely with the team in setting goals for the PWIDs and their caregivers.
- Working closely within an interdisciplinary team.
- Working in close partnership and collaborations with external partners and service providers across the health, disability and social settings.
- Clear and consistent documentation of encounters with PWIDs, caregivers, families, service providers and volunteers.
- Performing all of the above through home visits, clinic sessions, visits to service providers, teleconsults, phone calls and regular meetings.
Community
- Outreach and raising awareness amongst healthcare, eldercare, social and disability service providers.
- Working with the team in establishing a service ecosystem for PWID and their families.
- Working with the team in developing a clinical care protocol and training framework.
Requirements:
- Min Diploma / Degree.
- Need to be registered with Singapore Nursing Board.
- Understanding of and ability to perform holistic assessments of the nursing issues facing PWID and their caregivers within the biopsychosocial-environmental context.
- Ability to develop, implement and review nursing intervention plans in a systematic and person-centered manner.
- Understanding of the health, disability and social needs of PWIDs and their families.
- Understanding the resources available for PWIDs and their families.
- Ability to work well within a team.
- Ability to work well with external partners.
- Good communication skills.
- Competent in documentation.
