Staff Nurse needed for Case Management Officer (Renal) *WEST*#HZT

• Assess plan, implement and evaluate care of patients with renal disease
• Conduct teaching and counseling for patients and their families / carers
• Conduct / assist to conduct specialty nurse-led clinic for patients and their families where needed
• Co-ordinate patients’ access and treatment sequence where required
• Assist with overall coordination of renal replacement therapy
• Assist with placement of patients in dialysis facilities as required
• Deliver patient care (inclusive of education and counseling) according to the established standards of care and practice using patient care plan / clinical pathways
• Contribute to multidisciplinary patient management
• Apply understanding of prevention of medico-legal issues in relation to patient care
• Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services
• Demonstrate proper use and care of equipment
• Demonstrate proper and economical usage of consumables and supplies
• Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Managers, Doctors and Paramedical Staff
• Assist to review, develop and evaluate clinical pathways and / or other plans of care in consultation with Specialty Nurses, Nurse Managers, Doctors and Paramedical Staff
• Precept newly joined renal coordinators in delivery of specialty care where needed
• Participate in quality improvement activities
• Take on additional responsibilities or tasks as assigned by the Head of Division
• Maintain a clean and safe working environment

• Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science.
• Registered Nurse with at least 3 years of clinical experience
• Able to demonstrate clinical skills, knowledge and critical thinking in delivery of patients’ care.
• Possess good interpersonal and communication skill
• Computer literacy skills – MS Office (Word / Excel / PowerPoint)
