Sr Executive (Continuing and Integrated Care, Public Healthcare, Medical Sector) #HDC

Key Responsibilities and Duties

Specific (80%)

  1. Ensure efficient day-to-day operations support and co-ordination for the delivery of post-discharge home care services/community nursing services to achieve the hospital's strategic plans, targets and ensure business continuity (including hospital critical incidents and national emergencies such as pandemic outbreaks).
  2. Work closely with both internal and external (clinical and non-clinical) stakeholders to develop/improve workflows, processes and policies that are integrated, efficient and aligned with the relevant (clinical and non-clinical) governance framework/principles for hospital's Business-As-Usual operations and during hospital critical incidents and/or national emergencies such as pandemic outbreaks.
  3. Develop and monitor key performance indicators while ensuring resources are used optimally within approved funding/budget.
  4. Collect, analyse and report data/info required for administrative, planning and management decision making purposes.
  5. Assist to formulate/implement strategies and plans (including pilot projects) to steer the department towards better and improved post-discharge home care services/community nursing services while keeping abreast of new/emerging technologies/tools and patient/community needs.
  6. Support the development and successful implementation of IT systems and tools, including but not limited to the Next Generation Electronic Medical Record (NGEMR), for the delivery of post-discharge home care services/community nursing services.
  7. Provide support in the preparation, planning, tracking and review of the department's budget, including pilot projects (if any).
  8. Investigate and report on patient's feedback/complaints related to post-discharge home care services/community nursing services following the hospital's feedback management framework/principles.

General (20%)

  1. Provide administrative and secretariat duties for department and assigned committee meetings. Duties include (but are not limited to) minutes taking, circulation of agendas/reports/approved minutes and follow through of agreed actions.
  2. Support community and outreach-related programmes/activities, including but not limited to engagement of GPs, community partners, to facilitate the continuity of patient care at patient's home/within the community.
  3. Perform ad-hoc duties as and when assigned/required.

Job Requirements

  1. Min Degree in any discipline from a recognised institution with min 1 - 2 years of working experience.
  2. Have practical knowledge and experience in hospital operations will be an added advantage.
  3. Possess excellent communication (spoken and written), interpersonal and co-ordination skills.
  4. Have strong project management and administrative skills, together with, good critical thinking and problem solving abilities.
  5. Possess a positive, proactive working attitude and able to manage various tasks/projects concurrently with specific timelines.
  6. Enjoy working in a fast paced and dynamic environment both independently and as a team player, yet is organised and meticulous.
