As a Social Work Associate, you will be responsible for the following duties:

• Conduct assessments for financial assistance applications.
• Support patients with applications for national financial support programs.
• Aid the medical social worker in reviewing and evaluating cases.
• Collaborate with the medical social worker to provide brief social assessments.
• Gather and organize relevant information for case evaluations.
• Carry out home visits when required.
• Provide education and guidance to patients and their families.
• Contribute to quality improvement initiatives as needed.
• Perform any other tasks assigned within the team.

Requirements:

• Min. Diploma in Social Work or equivalent.
