The Community Resource, Engagement, and Support Team (CREST) serves as the first mental health touchpoint in the community, raising public awareness, promoting early identification of at-risk individuals, providing support and connecting individuals and caregivers to health and social care services.
CREST-PDS focuses on dementia; offering psychoeducation, coordinating community resources and facilitating early planning and management of the condition of individuals with dementia and their caregivers upon diagnosis over six months to a year.
Key Responsibility Areas
Assist Social Workers in client monitoring, care coordination and follow-up.
Service referral to community partners.
Collaborating with community partners and agencies.
Conduct client screenings.
Provide basic emotional support to clients and caregivers.
Promote mental health awareness to clients and caregivers.
Prepare resource materials for clients and caregivers.
Support CREST team in planning and executing outreach events, screenings, and client-related activities.
Perform administrative duties.
Undertake duties assigned by the Executive Director, Head and Team Leads.
Requirements
Experience:
Executive functioning skills – planning, coordinating, organizing and implementing.
Administrative skills – documentation, filing and report writing.
Passionate about working with seniors and their family members.
Qualifications:
Higher Nitec in Community Care and Social Services.
Diploma in Social Work/ Counselling/ Gerontology (Community Health & Social Care).
Others:
Required to travel and conduct home visits to different locations.
May require occasional work on weeknights and weekends.
