As a Social Work Associate (Multiple Settings), you will be responsible for the following duties:

• Conduct assessments for financial assistance applications.
• Support patients with applications for national financial support programs.
• Offer emotional support and guidance to both clients and their families.
• Collect and organize information for case assessment.
• Perform home visits when necessary.
• Participate in quality improvement projects when necessary.
• Any other duties within the team.

Requirements

• Possess a Diploma in Social Work or equivalent.
