Collaborative Working Environment
Responsibilities
Conduct mean testing for all relevant patients
Gather and document socio-financial information of patients and family members to facilitate intervention by medical social worker/ counsellor.
Provide administrative support to MSW(s)/ Counsellors and administration of client cases.
Assist in follow up phone calls and liaison work with family members on gathering of necessary documents to facilitate completion of means test, inpatient admission paperwork and financial assistance.
Update and maintain training manual for new staff.
Provide basic information and referral services (Inclusive of outreach services for Bereavement Care Programme & answering any phone calls or walk-in enquiries).
Assist in Departmental programmes and any other duties assigned by Supervisor/ Head of Department.
Participates in in-service education programme and follow-up to ensure optimum transfer of learning.
Requirements
Diploma in Social Work, Social Services, community development related disciplines.
Preferably with 1-2 years of working experience in health and/or social service sector.
Bilingual and ability to communicate in local dialects preferred.
Passion to serve and provide good patient care.
Good interpersonal skills and able to work independently.
Flexible, adaptable and able to work effectively in varied setting.
Good in analytical and interviewing skills.
With aspiration to pursue social work degree, preferably.
