The Opportunity:
We are looking for Senior / Staff Nurse (SOC- Diabetes Clinic) to join us to provide high quality patient care.
Job Responsibilities:
- Assess patients' social history, lifestyle, and dietary habits to provide tailored counselling and help patients better understand and manage their diabetes.
- Collaborate closely with the Endocrinologist to develop personalized care plans for effective diabetes management.
- Conduct comprehensive patient education on insulin injection techniques, glucose monitoring (via glucometer or continuous glucose monitoring), review of glucose profiles, and other diabetes-related counseling in both inpatient and outpatient settings.
- Support the clinic's quality improvement initiatives to achieve better clinical and process outcomes.
- Perform any other duties as assigned by the Reporting Officer.
Job Requirements:
- Diploma in Nursing or State Registered Nurse Certificate.
- Specialist Diploma in Diabetes Education or a Certified Diabetes Educator certification preferred.
- Minimum 2 years of relevant experience, with a focus on chronic disease management.
- Demonstrated experience in providing comprehensive health education to patients with diabetes.
- Excellent clinical judgment, critical thinking, and problem-solving skills to deliver patient care in line with established protocols.
- Proactive attitude, strong team spirit, and professional maturity.
