Conduct functional assessment to determine required therapy interventions
Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes
Formulate client-centered and SMART goals in collaboration with client and stakeholders
Document and communicate assessment results, therapy plans, goals and outcome measures to client and relevant stakeholders
Modify therapy plans based on client performance and motivation, safety, as well as therapy outcome measures collected and review continuity of care plans
Recommend assistive devices and equipment
Select appropriate outcome measures to enable evaluation of therapy progress and outcomes.
Provide client education by empowering clients/caregivers with methods to enhance health, mobility and functions through self-management strategies
Perform clinical administration and operations through development of clinical practice guidelines
Participate in continuing education, training and research activities
Provide clinical supervision and team development for support staff and students
2. Resource Management
Oversee inventory management process
Carry out routine disinfection and cleaning of equipment
Oversee monthly statistics and billing as per requirement
3. Clinical Quality and Safety
Oversee delivery of consistent and excellent clinical and service quality to residents and sponsors
Responsible for the clinical quality of all residents guided by the set key performance indicators
Co-lead quality improvement project(s) in the center(s)
Perform peer audits on compliance to clinical standards
Respond to clinical incidents in compliance with organization’s incident management guidelines
Monitor team’s clinical and non-clinical key performance indicators
Deliver training or in-service to colleague
4. Feedback Management
Assist in any investigation pertaining to complaints received
Escalate any unfavorable and unresolved feedback to Manager, Rehab
5. Other Duties
Assume any other additional responsibilities as assigned by HOD and Manager, Rehab
Maintain good communication skills with residents and sponsors
Requirements
Candidate must possess a recognised qualification in Physiotherapy and fully registered with AHPC, Singapore
At least 5 year(s) of supervisory experience
Applicants must be able to communicate effectively with a wide variety of people and cultures, maintaining a high level of quality service
Proficient in neuro and geriatrics rehabilitation preferred
Applicants must be willing to travel across Singapore
Ability to work effectively in team environment
Possess strong communication, and interpersonal skills
