Job Description

You will provide pharmaceutical care by working collaboratively with a multidisciplinary health care team to promote wellness, maintain current health, and/or intervene in acute or chronic illness.

Responsibilities:

1. Recommend and monitor patient drug therapy and provide drug information.
2. Identify, resolve, and prevent drug-related problems.
3. Interact with patients, family, and other health care providers to assure continuity of services and to achieve desired patient outcomes.
4. Ensure the accurate and timely distribution of pharmaceuticals, and provide education to patients, staff, and other health care practitioners.

Job Requirements

1. Degree/Master Graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council.
2. Pre-Reg turning Full registration are welcomed to apply.
3. Full registration with the Singapore Pharmacy Council.
4. Valid membership with local professional body/association is preferred where applicable.
5. Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage.
6. Working Hours: Normal Office Hours - 8.30am to 6.00pm, will be required to work half a Saturday from 8.30am to 12.30pm once every 4 weeks.
