- Conduct comprehensive psychosocial assessments of patients to identify needs, strengths, and resources.
- Conduct complex psychosocial assessments of patients and families, especially in high-risk or urgent situations.
- Facilitate patients and families in understanding and managing the medical condition.
Care Coordination and Planning:
- Lead psychosocial care planning with multidisciplinary healthcare teams, ensuring alignment with patient goals and resources.
- Collaborate with healthcare teams, including doctors, nurses, and therapists.
- Facilitate communication between patients, families, and medical staff to ensure that care plans are understood and followed.
- Coordinate care planning, ensuring a smooth transition from home to hospital or other care settings.
Resource Management:
- Identify and connect patients and families with community resources, such as financial assistance, house improvement, transportation, and support groups.
- Support patients' psychosocial needs, ensuring access to appropriate services and support.
- Assist with applications for public benefits and other financial resources.
Urgent Case Intervention:
- Provide urgent case intervention services for patients and families dealing with trauma, end-of-life issues, or sudden changes in health status.
- Work with multidisciplinary teams to ensure immediate and appropriate responses to urgent psychosocial needs.
Documentation and Compliance:
- Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in accordance with departmental policies.
- Ensure compliance with legal and ethical standards in social work practice, including confidentiality and informed consent.
Education and Support:
- Educate patients and families about healthcare systems and policies to empower them in their care decisions.
- Facilitate support groups and educational sessions for patients, families, and caregivers.
- Provide training and guidance to healthcare staff on psychosocial issues and resources.
Supervision:
- Provide supervision and mentorship to junior social workers and interns.
- Lead case discussions and case conferences to ensure high-quality service delivery.
- Serve as an expert on psychosocial aspects of care in team meetings and consultations.
Professional Development:
- Stay current with social work best practices, healthcare policies, and community resources through ongoing professional development.
- Participate in supervision, case conferences, and multidisciplinary team meetings to enhance service delivery.
Any other related duties as directed by the said reporting officers.
Requirements:
- Degree in Social Work or Master's Degree in Social Work or equivalent.
- Registered with Singapore Association of Social Work.
- Minimum 5-7 years of relevant work experience, preferably within healthcare settings.
Other Information:
- Ability to manage complex cases with minimal supervision.
- Strong leadership and mentoring skills to guide and develop junior staff.
