Senior/Clinical Coordinator (Contract, Adolescent Medicine Services)

Job Description:

1. Assist in monitoring the collection and input of data for clinical audit of service. Formulation and revision of guidelines, standards, and procedures. Proposing, redesigning and implementing strategies to improve clinical services.
2. Case management to provide and coordinate continuity of inpatient and outpatient care from initial triage to assessment, initiation of treatment till recovery or transition to adult services.
3. Conduct home visits as required.
4. Coordinate patient care and provide updates to multidisciplinary team and initiate case review and discussion.
5. Establish and maintain resources for psychoeducation of patients and families.
6. Establish rapport and build therapeutic alliance with patients and families for continuity of care across services and inter-sector collaborations.
7. Work closely with patients, families, and various services, coordinating discussions with community partners and schools. Conduct patient education and therapeutic groups.
8. Participate in research studies and quality improvement projects.
9. Co-facilitate and co-organise various support and therapy groups, workshops, and other activities.
10. Perform any other duties assigned.

Job Requirements:

1. Degree in Psychology/Social Work/Counselling OR Postgraduate/Graduate Diploma in Psychology/Social Work/Counselling with at least 6 years of relevant experience in areas of adolescent health OR
2. Master of Counselling or Master of Social Work or Master of Family Therapy with at least 5 years of healthcare or community experience in areas of adolescent health.
3. Good team player and yet able to work independently.
4. Strong interpersonal and communication skills.
5. Strong analytical, organising, and prioritising skills.
6. Possess a positive personality with high initiative and ability to solve problems and work with minimal supervision.
7. Work requires a broad knowledge of organization, communications, and program coordination.
