Senior / Care Coordinator, Population Health & Integrated Care

You will track and maintain records of all patients referred to the SWITCH (Sengkang Weight Improvement Therapy & Complete Health) and SUPREME (SUrgical PREhabilition for coMprehensive Enhanced recovery) programmes. These include demographic information, medical history and treatment plans. You will collaborate with the multidisciplinary team involving physicians and allied health professionals on the care coordination and case management of patients. You will also collect data on all aspects of patient treatments, administer surveys and incentives, manage educational workshops and talks, support research projects and prepare reports for publication. Additionally, you will facilitate meetings with various stakeholders, organise population health initiatives for the community and ensure that the programmes are in compliance with all relevant regulations and standards.

Requirements:

• Diploma with 8 years’ experience or Degree in a healthcare-related field, suchas nursing or health administration, with experience in a healthcare-related field
• Excellent analytical, organisational and communication skills
• Ability to work with diverse groups of people
