Primary Care Coordinator

Job Description:

1. Work closely with nurses to identify patients requiring counselling and support.
2. Collaborate with clinics to obtain informed consent for patient enrollment, referrals, and care coordination process within the primary care network.
3. Liaise with the service providers in scheduling ancillary service deployments.
4. Prepare the report after each deployment.
5. Track and submit monthly ancillary report.
6. Provide administrative support to the admin team in administering surveys and collating data for analysis and reporting.
7. Any other duties as assigned by supervisor.

Requirement:

1. Diploma in Healthcare Management, Counselling, Social Work, Business Management, or equivalent.
