Pharmacist

• Performed medication reconciliation to identify medication(s) taken prior to admission, drug related problems leading to current admission and documented it.
• Handled stocks and ordered supplies of medicines in unit of dose to the ward.
• Processed medication orders and packed in accordance with SOP.
• Responded to queries or drug information to patients, caregivers, healthcare professionals, students, and/or the public.
• Performed medication counselling, inhalers technique counselling, and warfarin counselling to the patients and/or caregivers.
• Reviewed and processed medication orders and provided a pharmaceutical care plan to the patient.

Requirements:

• Recognized Degree in Pharmacy.
• 3 years and above of relevant experience.
• Able to perform shift duties.
