Job Title: Medical Social Worker in various sites

Location: Singapore

Job Summary: The Medical Social Worker (MSW) provides comprehensive social work services to patients and their families to address the psychological, emotional, social, economic, and physical impacts of medical conditions. The MSW collaborates with healthcare professionals to ensure holistic patient care and support.

Key Responsibilities:

1. Case Management: Conduct assessments to identify patients' needs and develop individualized care plans. Provide casework, counseling, and therapeutic group work to patients and family members.
2. Counseling and Support: Offer emotional support, crisis intervention, and counseling to patients and their families. Assist with coping strategies and adjustment to illness or disability.
3. Resource Mobilization: Connect patients and families with community resources, financial assistance, and support services. Liaise with government departments, social service organizations, and private agencies.
4. Care Coordination: Participate in multidisciplinary team meetings, ward rounds, and case conferences to ensure coordinated and comprehensive care. Facilitate discharge planning and post-discharge support.
5. Advanced Care Planning (ACP): Assist patients and families with ACP, including decision-making for future care treatments when patients are no longer able to do so.
6. Documentation: Maintain accurate and up-to-date case records, including assessments, care plans, and progress notes.
7. Education and Advocacy: Provide psychoeducation to patients and families about medical conditions and available resources. Advocate for patients' rights and access to services.

Qualifications:

1. Bachelor's or Master's degree in Social Work from a recognized institution.
2. Registered with the Singapore Association of Social Workers (SASW).
3. Relevant experience in a healthcare setting is preferred.
4. Strong interpersonal and communication skills.
5. Ability to work collaboratively in a multidisciplinary team.
6. Knowledge of community resources and social services in Singapore.
