Job Role: Medical Social Worker


Working Hours: Monday to Friday, 8.15am to 5.30pm
1 Saturday per month, 8.30am to 12.30pm

Job Description:

  • Coordinates and involves in residents' case management through a social services approach.
  • Addresses psychosocial needs through care and counselling.
  • Assists staff members in understanding the significant social and emotional factors related to the residents’ health problems. Liaises with internal colleagues on matters/feedback relating to residents and fellow colleagues.
  • Participates in the development of the plan of care. Attends multi-disciplinary meetings with medical staff, nurses, and other healthcare professionals to discuss, propose, and coordinate care plans for residents.
  • Works with other health professionals significantly involved in the care of the resident.
  • Discusses long-term care issues with the resident and family and conducts ACP/PPC/EOC.
  • As a case admission coordinator, liaises and works with all referral sources, screens social background, plans, and coordinates case admission.
  • Advises residents and family members on chargeable fees and refers them for subsidies and financial aid if necessary.
  • Performs fieldwork involving visiting residents at wards and other social agencies to help residents alleviate or resolve their medico-social problems.
  • Prepares social and other related reports for file, management, and other related institutions/agencies.
  • Undertakes to keep well-informed on government guidelines on healthcare and social services available within the community.
  • Maintains case records on resident manual/computerized. The records kept include individuals who have been rejected by doctors for admission and inquiries from the public on MSW services.
  • Assists in conducting household means testing for residents and applications for MOH subsidy funding and other sources of financial assistance, e.g., MFEC and other government schemes.
  • Assists residents in applications for Medifund/Medifund Silver/FAS.
  • Assists in coordinating government payout, GovCash listing, and obtaining consent.
  • Advises defaulters in formulating repayment plans when requested by the finance department and other healthcare institutions or services.

Qualifications:

Degree in Social Work with clinical experience in a setting dedicated to the care of the older person or care management, and preferably, post-basic qualifications in gerontology.


Experience:

Equipped with experience in healthcare or long-term care settings, preferably working with the elderly. Strong assessment, counselling, and communication skills. Knowledge of relevant regulations.
